Portsmouth City Council - the ‘Council’ - has awarded a Contract for a service to provide information, advice and support to individuals diagnosed with a stroke or transient ischaemic attack, as well as their family and carers. The service will be provided to both newly diagnosed individuals when discharged from an acute setting and to existing stroke survivors within the community.
The Service will be delivered by an Information, Advice and Support Service Co-ordinator, who will help and assist the individual with information and specialist support which will empower them to select their own personal support packages thereby ensuring choice, independence and well-being. The Service aims to maximise health and well-being opportunities to achieve the best possible outcome.
The contract commenced on 1.10.2014 and will run for an initial term of 4 years but is extendable in increments to be agreed to a total maximum duration of 6 years, subject to performance and agreement of both parties.
The value of the contract is approximately 42 000 GBP per annum making a total maximum contract value of 252 000 GBP if the extension options are taken up in full.
