Herefordshire Council is undertaking an open tender process for the provision of a Stop Smoking Service and NHS Health Checks. This is a Part B Services Contract.
The Council is undertaking an Any Qualified Provider (AQP) procurement which is designed to support the development of Stop Smoking Services and NHS Health Checks in Herefordshire. It will provide funding to assist with the identification and proactive management of patients with modifiable risk factors for disease.
Services covered by this AQP procurement have been separated into 2 Lots:
Lot 1 - Stop Smoking Services:
The purpose of the Stop Smoking Service is to reduce the number of smokers by encouraging and supporting smokers to quit the habit and adopt a healthier smoke free lifestyle.
Lot 2 - NHS Health Checks:
The aim of NHS Health Checks is to identify individual risk of vascular disease (heart disease, stroke, diabetes, kidney disease and certain types of dementia) in the local population aged 40-74, for those not already diagnosed and provide appropriate advice and interventions to reduce or mange that risk.
