NHS Cornwall and Isles of Scilly and Kernow Clinical Commissioning Group (KCCG) are seeking to shape new care pathways for the treatment and care of patients with long term conditions that make innovative use of information technology. The commissioners are considering a telehealth service with the aim of improving the care of people with long-term conditions (LTCs) in their own homes. This will reduce the need for acute management of LTCs and reduce avoidable hospital admissions for people living with a LTC in Cornwall and Isles of Scilly.
A number of core objectives have been defined to ensure that the expansion of telehealth in Cornwall and Isles of Scilly fully exploits the potential of the technology to enable safe, effective and value-for-money care to be provided for people with long-term conditions in their own homes. These are to:
- procure an innovative telehealth and telecoaching service that can support up to 10 000 concurrent telehealth users and 10 000 concurrent telecoaching users,
- redesign and improve care pathways for long-term conditions to utilise the telehealth and telecoaching service as part of the mainstream management of these conditions,
- maintain and further develop good relationships with clinicians and make them aware of the benefits of telehealth and telecoaching for their patients,
- improve patient and public education and awareness around telehealth and telecoaching and its ability to enable greater self-care.
The expansion will create an at-scale, telehealth and telecoaching service and several options are currently under consideration for the required service model, the core outcomes of which are:
- patients and their carers who are better educated on how to improve the management of their health,
- patients who are empowered to take a more active role in their self-care, and have an increased their independence and better quality of life,
- a reduction in unnecessary emergency admissions through the provision of clinical intervention at an earlier stage in the patient pathway,
- reduced lengths of stay by enabling earlier discharge of patients from hospital,
- improved health outcomes, and quality of life for patients and their carers,
- greater confidence and security for patients in their own homes, through knowing that early warnings of health problems will be detected,
- increased care in the community,
- proactive case load management and visit prioritisation for community nursing teams,
- increased medication concordance.
NHS Cornwall and Isles of Scilly in conjunction with Kernow Clinical Commissioning Group (KCCG) are leading this review and seek to allow respondents to indicate their interest in providing all or any part of telehealth and telecoaching services, whether as a sole provider or as a joint venture.

Market Engagement Event:
Providers will be invited to attend a Market Engagement Event to be held in October 2012. The purpose of this event will be for potential providers and commissioners to;
- meet interested providers and understand their potential telehealth service offerings,
- introduce the NHS Cornwall and Isles of Scilly and KCCG Team,
- provide an overview of the project aims and aspirations,
- provide a networking opportunity for providers and commissioners,
- opportunity for questions.
Full details of this event will be cascaded to selected providers in due course.
