Sandwell's Lifestyle Service will recognise everyone's potential for making changes, for being more active, for maintaining a healthy weight and being supported during that process in order to improve the quality of life for individuals, families and communities.
There are three main service areas identified within the Lifestyle Service:
Local Area Single Assessment and Referral Service (LASARS) - Working closely with existing Council services, the service area will provide high quality and easily accessible intake, engagement and assessment services for adults and families who are either not active and/or are overweight/obese to enable them to access appropriate treatment and support focused on making sustained behavioural change. It will also seamlessly transfer service users to the intervention/treatment service.
Intervention / Treatment Service - The service area will sit within a wider lifestyle, health improvement system. The service will offer a range of physical activity and healthy eating options together with weight reduction and management options that are tailored around an individual or family's needs. Clear and seamless pathways will be established to move service users from the LASERS service, via successful completion of behaviour change interventions, into the maintenance support service.
Maintenance / Support Service - The service area will support individuals and families to sustain the changes achieved in habitual patterns of healthy lifestyle behaviour by helping clients and families to develop, work towards and achieve their personal, social and collective lifestyle goals and maintain them.
