The NHS Health Check Programme is a national Public Health programme that local authorities are legally responsible for providing to local people.
Everyone between the ages of 40 and 74, who is eligible for this programme will be invited (once every five years) to have a health check to assess their cardiovascular disease (CVD) risk and will be offered support and advice to help them reduce or manage that risk.
The provider will deliver an enhanced service model that delivers both routine (GP invites) and opportunistic (community and workplaces) health checks. The aim is to maintain high quality support to the highest risk patients while extending the coverage to at least 75 % to all those who are eligible for NHS health checks in Sandwell. The provider will manage a CVD team who will ensure the smooth running of the NHS Health Check Programme activities through management of a call and recall system and direct delivery of health checks at various settings. The Provider will also be undertaking subcontracting activities with other providers, undertake monitoring, quality control (training and audit) and undertake evaluation at all stages of the programme. The Provider will also manage data effectively through the procurement and management of an appropriate chronic disease management software tool that is compatible with all types of clinical GP practice systems and also ensure suitable adaptable software tools are in place to allow data to be transferred easily between different providers and at different settings.
